To ensure a smooth and beneficial visit, we ask that you complete and submit the below forms prior to visiting, as completing as much as you can before your appointment will save a significant amount of time in the clinic.   • List of prescriptions and/or over-the-counter medications, including dose and frequency, that you are currently taking
  • Contact information for your primary care doctor, and any specialists you see who we may need to request records from
  • Any relevant health records
